Clearinghouse Form
A prospective student-athlete must be certified by the NCAA Initial Eligibility Clearinghouse to participate for Concordia College as a freshman student-athlete. The Clearinghouse ensures consistent interpretation of NCAA initial-eligibility requirements for all prospects at all member institutions. To register for the NCAA Clearinghouse, please click HERE.

As a student-athlete, there are a few forms that must be filled out on a yearly basis BEFORE you can begin practice or NCAA competition.

1) As a student-athlete at Concordia College, you are required to take a physical examination. There is a separate physical examination form for incoming freshmen/transfer students and returning students.

2) A student-athlete at Concordia College must also provide proof of insurance. If your insurance is paid and provided for by your parents, guardian or legal representative, you must fill out the Insurance Form (Parent Version). If you pay for your own insurance, then you must fill out the Insurance Form (Student Form).

3) A student-athlete at Concordia College must also provide an Emergency Contact Information Form. There is a blank form to use if you provide your own insurance and there is a separate form to use if you purchase the College's insurance. It contains the information of the College's insurance provider.
