Athletic Forms
As a student-athlete at Concordia College, whether new or returning, there are a few forms that must be filled out on a yearly basis BEFORE you can begin practice or NCAA competition. Please return all forms to the Department of Athletics.
1) Health Form - As a student-athlete at Concordia College, you are required to take a physical examination before you can begin practice or NCAA competition.
2) Insurance Form (Parent Version) - A student-athlete at Concordia College must also provide proof of insurance. If your insurance is paid and provided for by your parents, guardian or legal representative, you must fill out the Insurance Form (Parent Version).
OR
Insurance Form (Student Version) - If you pay for your own insurance or purchase the Concordia College insurance, then you must fill out the Insurance Form (Student Form).
3) Emergency Contact Form (Personal Insurance) - A student-athlete at Concordia College must also provide an Emergency Contact Information Form. If you, your parents or legal guardians have insurance for you, please fill out this form.
OR
Emergency Contact Form (College Insurance) - If you purchase insurance through Concordia College, please fill out this form.
4) Copy of Insurance Card - If you, your parents or legal guardians provide insurance for you, we must have a copy of you insurance card, both front and back.
